On 21 January 2010, 30 April 2010, and 23 February 2012, based upon patient’s complaints of permanent injury by Jihad Kaouk, a surgeon at the “world class” Cleveland Clinic the Centers for Medicare/Medicaid Services (CMS) found:

Specifically cited in CMS Report of 01/21/2010 as a result of Patient inquiry:

a. 482.13(a)(2)(iii) PATIENT RIGHTS: NOTICE OF GRIEVANCE DECISION

At a minimum: In its resolution of the grievance, the hospital must provide the patient with written notice of its decision that contains the name of the hospital contact person, the steps taken on behalf of the patient to investigate the grievance, the results of the grievance process, and the date of completion.

This STANDARD is not met

b. 482.24 MEDICAL RECORD SERVICES

The hospital must have a medical record service that has administrative responsibility for medical records. A medical record must be maintained for every individual evaluated or treated in the hospital.

This CONDITION OF PARTICIPATON is not met

c. 482.24(b) FORM AND RETENTION OF RECORDS

The hospital must maintain a medical record for each inpatient and outpatient. Medical records must be accurately written, promptly completed, properly filed and retained, and accessible. The hospital must use a system of author identification and record maintenance that ensures the integrity of the authentication and protects the security of all record entries.

This STANDARD is not MET

d. 482.24(c)(2)(v) CONTENT OF RECORD - INFORMED CONSENT

[All records must document the following, as appropriate:] Properly executed informed consent forms for procedures and treatments specified by the medical staff, or by Federal or State law if applicable, to require written patient consent.

This STANDARD is not met

CMS tells CEO Cosgrove, “We have determined that the deficiencies cited are significant and limit your hospital's capacity to render adequate care and to ensure the health and safety of your patients.

CMS is removed from deemed status for serious violations and placed under state oversight.

Castle Connolly has no credibility. After presenting Mr Connolly the "Centers for Medicaid Medcare Services" (CMS) reports of investigative findings of multiple violations against Jihad Kaouk which have resulted in the permanent injury to many patients, Mr Connolly continues to give Jihad Kaouk a 1% rating. These rating have no credibility and are invalid. As one previous writer stated, if your doctor is on this, run the other way.

Two of the top orthopedic surgeons listed for San Francisco are over 70 and one of them is either dead or retired (Callander). How old is this information?

What a friggin joke this website is. Top doctors in the U.S. Ya, right. Larry Barnett has been sued for screwing up a total knee replacement. Dr. William McMaster I recall amputated the wrong leg of a Sheriff at St. Joe's about 5 -9 years ago. Get "real referrals" from freinds and patients and don't go by this crap. Total B.S. So what do they do? Have all they kids; neighbors; family; etc. write in and vote them the best doctor in the country. Pleeeeez, don't believe this nonsense. It's almost safe to say, if they are recommended on a website, you probably need to "run for the exit". Adios!

Jihad Kaouk, ranked in the top 1% on this site, is currently under investigation by the state medical board of Ohio, does not possess the board certification claimed, was denied board certification, has had multiple lawsuits filed for egregious permanent injury to patients, including life threatening injuries, was denied a medical license in the state of Iowa for not possessing the minimum requirements. Perhaps Castle Connolly would like to explain this misleading error. Perhaps Kaouk is one of many uncertified surgeons on this list responsible for devastating injuries to their patients. Shame on Castle Connolly, US News, Jihad Kaouk, and the Cleveland Clinic.

The one thing I did find helpful about this site is that I've been trying to narrow the list of doctors by specialty. The only thing this website has been helpful with is the ratings on the hospitals and being able to view what a doctor specializes in. In seeking a good Endocrinologist, it has been very time consuming trying to pinpoint which Endocrinologist specializes in specific thyroid diseases as some specialize in Diabetes, yet others have their specialty in cholesterol, bone loss etc. trying to find an Endocrinologist who specializes in Thyroid and Parathyroid disorders, hormonal disorders has been quite difficult and in this area, I give credit great credit to this website for making my search easier.
