12 billion of that increased cost can be conservatively chalked up to inflation. Unless medical related expenses do not follow the same inflation formulas as other goods and services?

Um, the price of everything has at least doubled in the last 18 years (food, fuel, labor, cars, houses, etc etc). It's called inflation (i.e. the amount of U.S. dollars in circulation).
