I would agree fully with Dr.Henry Black that it is precarious to push the blood pressure control to the back burner in particularly with diabetic patients as most of them are diagnosed late. Life-style changes are not adhered by all the patients rigorously. I would consider only life-style measures in the following patient who fulfill the below criteria:
1. Newly diagnosed diabetes (recent screening was negative )
2. Borderline Blood Pressure (stage I)
3. Correctable risk factors such as- obesity, smoking, high salt intake, physical inactivity, stress, dyslipidemia
4. Absence of microalbuminuria, diabetic retinopathy, and CAD
5. Absence of family history of CVA and End -Stage kidney disease
6. Patient can be relied upon to follow up closely.
7. A motivated patient who will persist with life-style change religiously.
