I had a telephone program plus printed material and instructions at a single, two-hour group meeting. A blood pressure cuff and scales were provided, and a variety of symptoms were charted on a form. Telephone contact decreased as compliance was demonstrated, plus the human on the other end was well-trained and compassionate, giving straight information while remaining approachable. I have had no further problems, and have been able to relax some of the restrictions as my situation was not chronic.
