Table: Highest and Lowest Hospital Readmission Rates

I would like to get in touch with the folks from the lowest readmission rate hospitals to see what they are doing that we don't do.

I am just starting a job as a Discharge Coach and would be very interested as to why each of these hospitals have either a high or low rate of readmission. Perhaps this would be material for a future article.

Rankings are useful only if the substance of the hospital practice patterns are also disclosed. Often it is assumed by the public that a high re-admit rate related to poor performance. This may be the case, but the reader must consider the risk for re-admission brought to the hospital based on the acuity of the patient. If a hospital is a specialty hospital that performs procedures on patients of high risk, such as transplants, burns or involved orthopedics, the re-admit rate may be mis-leading. Hospital size also relates to re-admit metrics as does location.
